Extended-spectrum beta-lactamases (ESBL) are bacterial enzymes that are responsible for resistance to penicillins and cephalosporins of the 3rd and 4th generations within the order Enterobacterales. Most ESBL producers are sensitive to cephamycins and carbapenems, although phenotypic resistance to the latter can arise through the combination of ESBL and loss of porin (AmpC).
For phenotypic confirmation of ESBL production in Enterobacterales in accordance with EBM 7-2018 for the extended antibiogram (21774), cephalosporin test disks are used in combination with clavulanic acid. EUCAST recommends (as of Q4-2019) the use of combination test disks for phenotypic confirmation of ESBL production in Enterobacterales with the following load: cefotaxime 5 µg +/- clavulanic acid 10 µg ceftazidime 10 µg +/- clavulanic acid 10 µg.
